Frequently Asked Questions
Please provide the following information:
- General Information: name of organization, address, phone number, title of contact persona, and contact person’s email address
- EIN number and documentation for the non-profit organization affiliated with the application (i.e. W-9 and IRS 501 (c)(3) determination letter (if applicable)
- Information containing the specifics of the sponsorship request and sponsorship levels. You will be asked to upload a PDF/Word document with this information.
We kindly ask for a minimum of eight weeks from the submission date to fully review each request.

We generally do not support requests that the support the following:
- Those directly benefiting an individual
- Individual sports teams
- Organizations not in our service area
- Organizations that discriminate based on race, color, sex, gender identity/expression, religion, sexual orientation, national origin, age, or disability.
By providing us with a W-9 form, it gives us a clear picture of your business needs for tax reporting. This helps our accounting department keep track of any 1099 forms that require reporting to the IRS.
For the W-9 form to be accepted, we kindly ask for the following:
- Please use the current year’s W-9 form and complete the following sections: Lines 1 – 3b, line 4 if you have an exemption, lines 5 and 6, Part I with either the EIN or SSN and then include a signature and date in Part II.
- If using a 2018 W-9 form, please complete the following sections: Lines 1 – 3, line 4 if you have an exemption, lines 5 and 6, Part I with either the EIN or SSN and then include a signature and date in Part II.
- W-9 is dated within the last 12 months.
